Consultations are by appointment only – there are no drop-ins. After booking an appointment please complete the following form and fax it to the number below or bring it with you to the appointment. Bring a list of any medications and/or supplements that you are taking or bring them with you. Allow one hour for your first consultation. Follow-ups are on an as-needed basis and are typically 30 minutes.
If you have copies of pertinent lab results please bring them with you to your appointment.
If you have been diagnosed with cancer, it would be helpful for you to deliver or fax your pathology reports before your first appointment.
